How to Enter:

1. Print out the entry form. (Select for Entry Form)
   Note: Entry Form is in PDF format. If you don't have Acrobat Reader you may download the free software. (Download Acrobat)
2. Fill out the entry form. Incomplete entry forms will not be processed.
3. Include all signatures for team members. Parent or guardian signatures are required if player(s) are under 18 years of age. Teams will not be registered without these signatures.
4. Mail your completed entry form and an entry fee of ($60.00 per team) to the address below.
5. Your team captain will receive written verification along with information about the tournament, rules and other events.

Cost to Enter: A nonrefundable entry fee of $60.00 must accompany your entry form. You can pay by check or money order. DO NOT SEND CASH.

Registration: Entry Form must be mailed (Postmarked) no later than the deadline.
